1. You can download the forms or wait for the postage-paid application. Forms are in English and Spanish. 

In Section 1, put a check mark for "temporary illness or physical disability." This rule was put in place by Governor Cuomo to enable people to receive an absentee ballot for the June 23rd Primary. It is not a statement on your health or well-being.

In Section 2, put a check mark for “Primary Election only.”

B. Send the form by mail, fax or email to the Westchester Board of Elections.
