1. Fill out the Application

You can apply for the ballot in 1 of 2 ways.

1. Apply online here: https://nycabsentee.com/absentee

On the second page, select “temporary illness or physical disability” as the reason for requesting the absentee 

2. Fill out the form and mail, email, or fax the form instead.

A. You can download the forms or wait for the postage-paid application. Forms are in English , Spanish, Chinese, Bengali, and Korean.

In Section 1, put a check mark for "temporary illness or physical disability." This rule was put in place by Governor Cuomo to enable people to receive an absentee ballot for the June 23rd Primary. It is not a statement on your health or well-being.

In Section 2, put a check mark for “Primary Election only.” 

B. Send the form by mail, fax or email to the NYC Board of Elections.

2. Vote
